"Quadriceps Trigger Points are the primary source of knee pain" Clair Davies, The Trigger Point Therapy Workbook, 2nd edition. Arthritis of the knee, ligament injury and deterioation of the joint cartilage are more often than not the medical diagnosis for knee pain. Knee (and hip) replacement surgery is frequently carried out all too quickly when in reality the pain is nothing more than referred pain from trigger points in the muscles of the thigh.
TRIGGER points and knee pain
There are many muscles that make up the thigh which, if loaded with trigger points, can cause a host of problems from knee pain, to hip pain and even groin pain. Then there are the hamstrings, which if containing trigger points can cause pain up into the buttocks as well as down into the back of the knee. However, it is without doubt the muscles of the quadriceps, know as the quads, that are the major contributors to knee pain.
REctus Femoris
The Rectus Femoris muscle is found on the front of the thigh and runs from the hip bone down to the knee. The primary trigger point refers pain deep into the knee (as shown), often feeling like it is under the kneecap. The connection from cause to symptom is often unsuspected as the trigger point lies far removed from the site of pain. A second trigger point lies just above the knee and causes a deep ache above the knee itself. Both trigger points can make the knee stiff and weak.
THE VASTUS INTERMEDIUS AND VASTUS MEDIALIS The Vastus Intermedius lies hidden beneath the rectus femoris trigger points in which are usually felt in the mid thigh. Pain may increase dramatically when climbing stairs. The Vastus Medialis is the muscle forming the large bulge toward the inner side of the thigh, above the knee. As the muscle name suggests pain is often felt on the medial border of the knee. These trigger points can make the knee weak, to the point that a bucking knee is often their signature. Arthritis, ligament damage and tendinitis are often the misdiagnosis given when in fact it is simply trigger points that are causing the pain. This becomes obvious when treatment of these trigger points shows a huge reduction or total cessation of knee pain!
vastus Lateralis
The Vastus Lateralis covers the whole of the outside of the thigh. It is the largest of the quadricep muscles and can be a source of pain in many areas form the hip, outer thigh to the knee itself. It is notoriously painful to treat and harbours active (pain producing) or latent (pain only on palpation) trigger points in almost everyone I have treated. Trigger points in this muscle are often mistaken for pain and tightness in the IT band, commonly experienced by the actively athletic. While tightness within the IT band is quite possible, assessment of trigger points in the Vastus Lateralis, together with the Tensor Fasciae Latae and Gluteus Maximus, two muscles which join to form the IT tract, more often than not results in a significant reduction of hip, thigh and knee pain.
